Group plan options consist of programs that are fully-insured, level-funded, self-funded, individual coverage with a health reimbursement arrangement (ICHRA), or those providing only minimum essential coverage (MEC).

A medical plan may feature HMO, EPO, or PPO designs.  Often, these plans are combined with a Health Reimbursement Arrangement, allowing clients to assist members with their plan deductible when necessary. Additionally, a qualified high deductible health plan is associated with a Health Savings Account that can be funded by both employees and, in some cases, the client.
